Tenorio Multi-Purpose Center in Susupe. All of those interested in availing of consular services are advised to prepare the following requirements:<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\"><b>A. FOR PASSPORT RENEWALS<\/b><\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">Due to the capacity limits of our passport processing equipment, we can only process up to 360 applications over the three-day mission;<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">Please note that only the 360 applicants who had pre-registered during the last consular mission in January 2015 will be processed during this mission;<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">The 360 applicants on the official list are each assigned a specific date and time slot;<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">Make sure that you appear during your scheduled processing time; otherwise you may have to wait until a slot opens up;<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">Those who have to renew their passports and who are not on the official list of 360 applicants to be processed for this mission can still come to pre-register and submit the necessary requirements;<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">Those who pre-register during this March mission will have their applications processed during the next consular mission;<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">Per department guidelines, the Consular Team cannot extend expired passports unless the holder will be simultaneously processed for renewal;<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">The Consulate General is well aware of the issues faced by our nationals who cannot renew their passports and we are working on solutions to alleviate their situation;<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">The official list of those who have appointment will be announced on March 20, 2015;<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">Requirements for renewal of GREEN Philippine passport:<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">1. Payment of ePassport Fee ($60)<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">Requirements for renewal of BROWN\/MAROON Philippine passport<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">1. Accomplished ePassport Application form<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">2. Photocopy of data page of passport<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">3. Notarized Affidavit of Loss ($25)<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">6. Original passport<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">7. Payment of Lost Passport Fee: $90 (Green\/Maroon) or $150 (ePassport)<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\"><b>B. REGISTRATION FOR OVERSEAS VOTING<\/b><\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">All citizens of the Philippines who are abroad, at least 18 years of age on the day of the elections (May 9, 2016), and who are registered overseas voters, may vote for President, Vice President, Senators and Party-List Representatives.<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">Personal appearance is necessary as the voter registration process requires the intake of a person\u2019s biometric data. Applicants should present proof of Philippine citizenship, such as a valid Philippine passport or NSO-issued certificate of birth.<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\"><b>C. FOR CIVIL REGISTRY AND NOTARIAL SERVICES<\/b><\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">All applications for civil registry and notarial services will be processed on a first come, first served basis. Applicants are advised to prepare the originals and copies of the required documents as indicated below:<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">Report of Birth ($25)<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">1. Original Birth Certificate + five copies<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">2. If applicable, Original Marriage Certificate of Parents + five copies<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">3. Valid Passport of Parents + five copies; If Passport is expired, valid government issued ID + five copies<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">4. US Naturalization Certificate of Parent + five copies, if parent is already a US Citizen at the time of application<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">5. Affidavit of Delayed Registration of Birth ($25), if filed more than one month from birth of child<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">6. For Illegitimate children using surname of father, Affidavit to Use Surname of Father ($25)<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">7. Duly accomplished ROB Form, signed in front of consular officer<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">Report of Marriage ($25)<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">1. Original Marriage Certificate + five copies<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">2. Original Birth Certificate of Filipino spouse + five copies<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">3. Valid Passports of both parties + five copies each; if passport is expired, valid government issued ID + five copies<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">4. 2&#215;2 photo (white background) of each of the spouses<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">5. For parties with previous marriage\/s:<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">a. For US citizens, Final Decree of Divorce + five copies<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">b. For Filipino citizens, Annotated Marriage Certificate from NSO showing details of Annulment + five copies OR Judgment of Philippine Court Recognizing Final Decree of Divorce issued by a foreign court<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">c. For widows\/widower, Death Certificate of deceased spouse<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">6. Affidavit of Delayed Registration of Marriage ($25), if filed more than one (1) month from celebration of marriage<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">7. For those whose spouses are non-Filipinos, duly accomplished CFO form<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">8. Duly accomplished ROM Form signed in front of consular officer<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">Report of Death ($25)<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">1. Original Death Certificate + copies<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">2. Duly accomplished ROD Form<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">Acknowledgements (SPA, General Power of Attorney, Contracts), Jurat (Affidavits)<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">1. Valid Passport or government issued ID (i.e., driver&#8217;s license) of signatory + copy<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">2. Must sign document in front of consular officer<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">3.
