Students will not be officially registered until all required documents have been submitted. Incomplete registration will not be accepted.<\/p>\n<p>Requirements include a completed registration form with attached PSS policy and permission form; copy of birth certificate; valid school entrance health certificate;  valid copy of school accident\/health insurance\/Medicaid; valid copy of both parents\/guardian(s) I.D.; copy of legal adoption\/guardianship issued by court (if applicable); transfer out documents from previous school; for Kindergarten students, Head Start completion certificate (if applicable).<\/p>\n<p>Registration forms are available at our main office. Documents can be submitted to the office on Mondays-Fridays between 1pm and 4:30pm.
