Entry forms are available at Las Vegas Discount Golf &#038; Tennis and the American Memorial Park. Players are reminded that entries for the doubles and juniors are due today, May 15 at 6:30 p.m. and for the singles and junior doubles by May 21 at 3.00 p.m..<\/p>\n<p>On May 19-21, mixed doubles, doubles, and junior singles will be held.  On May 27-29, men&#8217;s and ladies&#8217; singles and junior doubles will take place.  The categories in men&#8217;s singles and doubles are 5.0, 4.0, 3.0 and over 4.0, while in the women&#8217;s singles and doubles are 5.0 and 3.0.  The mixed doubles categories are 5.0 and 3.0.
