Deadline to submit Form SF-200

The Settlement Fund is requesting all members to submit their 2020 Annuity Recipient Information Update form (Form SF-200). The form can be obtained online or at the Settlement Fund office. You must attach the following documents to the Form SF-200:

A copy of a valid photo identification, such as a driver’s license, passport, or other ID issued within the past five years;

If you are a retiree, a certified copy of a divorce decree and final judgment, adoption decree, or other certified orders if none on file; and

If you are a disability annuitant or a surviving spouse, a copy of the 2019 Income Tax Return is required; and for surviving spouses not required to file a tax return, you must submit a letter of justification regarding the non-filing and Affidavit of Surviving Spouse (Form SF-8).

All forms are available for download at and at the Settlement Fund office on Capital Hill.

The final deadline to submit the 2020 form and accompanying documents is April 15, 2021. Failure to comply, including failure to provide complete information or documents, will result in a suspension of benefit payments until such time that a member is in full compliance.
